Espanola and Area Family Health Team Invited to Present at The Power of Primary Care Summit

Espanola, ON — The Espanola and Area Family Health Team was proud to present at the Association of Family Health Teams of Ontario's Power of Primary Care Summit on October 16 in Markham, Ontario.

The team was invited to share its Primary Care Access Clinic, an innovative model developed to support residents without a regular primary care provider. The clinic uses evidence-based pathways and team-based care to connect patients with preventative services, chronic disease management, and ongoing healthcare supports while they work toward attachment.

The presentation highlighted how the model has helped move patients off the Health Care Connect waitlist and into active care, while allowing healthcare professionals to work to their full scope.

As primary care organizations across Ontario search for solutions to improve access, the Espanola Family Health Team was proud to share how a small rural clinic is making a meaningful difference and demonstrating a model that could support patients in communities across the province.
