Family Health Teams are health care organizations that include a team of family physicians, nurse practitioners, registered nurses, social workers, dietitians, and other professionals who work together to provide health care for their community. Family Health Teams provide more service and a wide range of health options, especially for people who don't have a doctor.
Family Health Teams ensure that people receive the care they need in their communities, as each team is set-up based on local health and community needs. They focus on chronic disease management, disease prevention and health promotion, and work with other health care organizations, such as public health units and Home and Community Care.
Our team continues to evolve as the health of our population changes. The Espanola & Area Family Health Team commenced operating in 2007. In addition to our 5 Family Physicians, the FHT includes: 3 Nurse Practitioners, 3 Registered Nurses, 1 Diabetic Nurse Educator, 4 Registered Practical Nurse, 2 Registered Dieticians, 2 Registered Social Workers, an Addictions Navigator (RAAM), a Manager and a team of Medical Receptionists. Each FHT is unique and has flexibility to design programs to suit the needs of its patients. Our team continues to evolve as the health of our population changes.
